Requirements

* Certificate of Death signed by a Physician attesting the cause of death and the embalmer of the funeral provider

* Processing Fee – (Burial P 50.00, Transfer P 100.00, Cremation P 500.00)

Steps

1. Submit requirements except processing fee at Civil Registry Office. Registration Officer will verify the requirements/documents and prepares order of  payments

2. Pay the processing fee at City Treasurer’s Office Window 14 – 18. Secure Official Receipts

3. Presents Official Reciepts at Civil Registry Office. Registry Office will then assigned registry/document number for your document and wait for your certificate to be release.
