1. Go to City Health Office at 3rd Floor of the City Hall building. Bring the following requirements mention above. And secure order of payments form.
2. Pay the processing fee at Treasurer’s Office at Window 14-18.
3. Go back to City Health Office. Submit O.R. for release of your Health Certificate
